Insurance and Fees

Monarch Endocrinology participates with Medicare, and major Commercial plans, including Connecticare, Anthem BCBS, Aetna, Care Partners of Connecticut, Cigna, Harvard Pilgrim Healthcare, Oxford, Health New England and United Healthcare.  We also participate with certain Tufts and Wellcare Medicare Advantage Plans.  We do not participate with Medicaid at this time. 

This list is not exhaustive, and also is not a guarantee of coverage.  Please contact your Health Insurance carrier to verify coverage.  

We accept all major credit cards including Visa, Mastercard and Discover.  We also accept cash and personal checks.  

No Shows:

Missed Appointment/Late Cancellation: $75

Out of Network and Self Pay:

New Patient: $250

Established Patient: $150

Two missed appointments or late cancellations will result in termination of the physician-patient relationship, and discharge from the practice. 

Inappropriate, rude, threatening, or abusive behavior directed at any members of our staff will result in termination of the Physician-Patient relationship and discharge from the practice.
